Aflac Plans
Accident Plan
Benefits are payable for a covered person’s death, dismemberment, or injury caused by a covered on- or off-the-job accident.
Specified Health Event Plan
This plan pays a first-occurrence benefit, hospital confinement benefits, and continuing care benefits for heart attack, coronary artery bypass surgery, stroke, end-stage renal failure, major human organ transplant, major third degree burns, coma, and paralysis.
Cancer Plan
This plan may defray cost expenses not covered by your medical plan, including deductibles, out-of-network specialists, experimental cancer treatment, and ambulance, transportation, and lodging benefits.
Enrolling or Making Changes
New Hires
- Newly hired faculty and staff may enroll in an Aflac Plan(s) by calling (336) 991-4590 or visiting the Aflac website (Plan Number: HK989) within the 30 day new hire period.
Throughout the Year
- Following a qualifying event, faculty and staff may enroll in an Aflac Plan(s) by calling (336) 991-4590 or visiting the Aflac website (Plan Number: HK989).

New York Life Plans
Life Insurance
Wake Forest provides the following basic coverage:
- If salary is less than $50,000: 1.5x salary rounded to next highest thousand, up to $50,000
- If salary is $50,000 or more: 1x salary rounded to next highest thousand, up to $100,000
Additionally, you may elect one to five times your salary in voluntary coverage, up to $500,000.
Dependent Life Insurance
The voluntary dependent life insurance plan is a group term life plan that covers a spouse and/or dependent child(ren) in the event of their death. You may choose $10,000 or $25,000 coverage for your spouse and/or $5,000 or $10,000 for your child(ren).
AD&D Insurance
If your death is ruled accidental, the value of your basic life insurance policy is doubled. You may also elect up to 10 times your annual salary in voluntary AD&D insurance, up to $500,000, in $10,000 increments.
Enrolling or Making Changes
Life Insurance
New Hires:
- If you are a full-time faculty or staff member, you are automatically enrolled for basic life insurance on the first of the month following your date of hire.
- You will have 30 days to elect additional voluntary coverage without providing a Medical History Statement* by completing enrollment in Workday.
*Coverage may be the lesser of three times your base salary, or $400,000, without a Medical History Statement.
Throughout the Year:
- Log into Workday to elect coverage throughout the year.
- Email AskHR@wfu.edu to request the required Medical History Statement; complete and submit this form to New York Life.
- If the insurer approves your coverage, Human Resources will set up your payroll deduction.
Dependent Life Insurance
New Hires:
- Elect voluntary life insurance for yourself, as this is required before electing dependent life insurance.
- You will have 30 days to elect coverage without providing a Medical History Statement by completing enrollment in Workday.
Throughout the Year:
- Elect voluntary life insurance for yourself, as this is required before electing dependent life insurance.
- Log into Workday to elect coverage throughout the year.
- Email AskHR@wfu.edu to request the required Medical History Statement; complete and submit this form to New York Life.
- If the insurer approves your coverage, Human Resources will set up your payroll deduction.
AD&D Insurance
New Hires:
- If you are a full-time faculty or staff member, you are automatically enrolled for basic AD&D insurance on the first of the month following your date of hire.

Unum Whole Life Insurance
Benefits
- The cost will not increase with age.
- The benefit coverage amount does not change.
- The policy earns interest or a cash value at a guaranteed rate of 4.5%
- You may borrow from the cash value.
- It has a living benefit that allows an early payout of the death benefit, if certain criteria are met.
- You may be able to use the death benefit to pay for long-term care.
Coverage Options
- Faculty and staff can purchase $10,000, $25,000, $50,000, or $75,000 of coverage for oneself.
- Spousal individual coverage, ages 15 – 80, $10,000, $25,000, $50,000, or $75,000, even if you do not purchase coverage for oneself.
- Child(ren) or grandchild(ren) individual coverage $10,000 or $25,000, even if you do not purchase coverage for oneself.

- Newly hired faculty and staff may enroll in Unum Plans by logging into Workday, clicking the Menu icon, clicking Benefits and Pay, and under Suggested Links, clicking Unum Voluntary Benefits Enrollment.
Throughout the Year
- Following a qualifying event, faculty and staff may enroll in Unum Plans by logging into Workday, clicking the Menu icon, clicking Benefits and Pay, and under Suggested Links, clicking Unum Voluntary Benefits Enrollment.

Genworth Plan
Important Change Notification: Genworth Group Long-Term Care Insurance Discontinuation of New and Additional Coverage
Effective November 1, 2024, Genworth Life Insurance Company will discontinue offering new and additional coverage under the Wake Forest University’s Group Long-Term Care Insurance Policy (Policy #17295). Applications received by Thursday, October 31, 2024 will be underwritten in accordance with Genworth’s normal procedures. Any applications that are received after Thursday, October 31, 2024 will not be accepted or considered. Visit Genworth for information on enrollment.
For current policyholders, coverage will continue in accordance with the Continuation Coverage provision in the certificate, as long as premiums are paid on time, subject to the “Premiums and Renewal” section of the certificate. Long-Term Care Benefits
This plan covers long-term care expenses for services received at home, in the community, or in a nursing facility.
Age-Based Premiums
Long-term care insurance is age-based, and the premiums will be determined during the application process.
Portable Coverage
Coverage can move with you if you retire or otherwise leave the University.
