Spousal Surcharge

Wake Forest University Reynolda Campus has a medical plan spousal surcharge. This surcharge will apply if your spouse has access to medical coverage through an outside employer but enrolls in the Wake Forest plan.

Surcharge Waiver

The surcharge will be waived if any of the below conditions apply to your spouse. If they:

• are not eligible as an employee for medical coverage through their employer
• work for an employer that does not offer medical benefits
• do not work
• are self-employed
• work at Wake Forest University Reynolda Campus

Surcharge Will Apply

The surcharge will apply if any of the below conditions apply to your spouse. If they:

• are eligible for medical coverage through their employer, but is covered on the Wake Forest medical plan
• are eligible and enrolled for medical coverage through their employer, and covered on the Wake Forest medical plan

Surcharge Cost

• Exempt Faculty and Staff = $75 per month (pre-tax)
• Biweekly Staff = $34.62 per pay cycle (pre-tax)
