Achieving personal health goals is easier with the right support. Wake Forest University partners with Tria Health to bring professional, personalized health care directly to the campus community. Plus, Tria is a fully covered, confidential benefit available through your health insurance, costing those eligible nothing.

While managing a chronic condition and keeping track of multiple medications can feel overwhelming, getting started with Tria Health is simple. You’ll partner with an experienced pharmacist to review your medications, lifestyle habits, and preventive services to ensure everything is working safely, prevent side effects, and find ways to save you money. Together, you’ll create a custom care plan — and if you’re managing diabetes or high blood pressure, you’ll even receive a free blood pressure cuff, blood glucose meter, and testing supplies.

Tria Health also supports overall lifestyle transformations through its Choose to Lose program, which offers the opportunity to speak with a health coach for expert advice on how to achieve weight-loss goals. Qualified participants receive access to a nutrition tracking app alongside a designated health coach and clinical pharmacist to help maximize energy and wellbeing.

With a compassionate team and confidential consultations, you can also take the first step towards improved wellbeing with Tria. Trained pharmacists can support your mental health journey with confidential conversations, custom plans to center you and prevent issues, and professional support with tips on how you can better care for your mental health.

In order to register on Tria Health’s patient portal, you’ll need to provide your 10-digit Tria ID. Eligible members may have already received a letter and ID card mailed by Tria Health.
