About Accommodations at WFU
- Reasonable Accommodation: a reasonable modification or adjustment to a job, employment practice, or the work environment that makes it possible for a qualified employee to perform the essential functions of their position.
- Interactive Process: a collaborative dialogue between the employee/applicant, supervisor, and OIE (HR) office to determine appropriate accommodations.

Accommodations Request Forms
Employees or applicants may request accommodations using the following forms:
- ADA Medical Accommodation & Certification Request Form
- Pregnancy and Related Conditions Accommodation Request Form
Completed forms should be submitted to ADA@wfu.edu or faxed to 336.758.3021.

How to Request Accommodations
How the accommodations process works:
Step 1: The employee or applicant completes the medical or pregnancy accommodation request form.
Step 2: The ADA/Section 504 Coordinator reviews the documentation.
Step 3: Coordinator, the employee or applicant, their supervisor, and, as appropriate, the departmental and academic leadership discuss the request.
Step 4: The ADA/Section504 coordinator determines and approves reasonable accommodations, which are implemented within a reasonable timeframe.
Step 5: Accommodations are modified and adjusted as needed over time.
