This message was sent to active, full- and part-time faculty and staff from Human Resources.

Dear colleagues,

On Monday, campus received an email with important COVID-19 updates. The message included information about our plans to survey colleagues and collect COVID-19 vaccination records. If you haven’t yet been vaccinated, we need to hear from you too.

If you are vaccinated: Provide your vaccine record
If you’re among the majority of colleagues who have reported receiving the vaccine, we will ask you to verify the information you already supplied via SneezSafe and provide documentation of your COVID-19 vaccine to the university. This will allow us to confirm the rate of vaccination across campus. Providing your COVID-19 vaccine record is simple, you can either authorize Wake Forest Baptist Health (WFBH) to verify your record, or upload a copy of your vaccine record directly into Workday.

  • How do I authorize WFBH to verify my information? If you were vaccinated in North Carolina, you will be asked in the survey if you would like to provide authorization for WFBH to verify the information you entered.
  • How do I upload my vaccine record to Workday? If you were vaccinated outside of North Carolina, or prefer not to authorize WFBH to verify your record, additional instructions for uploading your vaccination record into Workday will be sent to you in the coming weeks.

If you are not vaccinated: It’s not too late
A fully vaccinated campus is the best path back to full operations and a reconnection with students and colleagues. The easiest way to find a vaccine provider near you is by visiting myspot.nc.gov. The COVID-19 vaccine is free for everyone – with or without insurance – and many local pharmacies offer same-day appointments and walk-ins. Staff members may schedule their appointment during working hours, and should inform their supervisor of time away from work for this appointment.

Plus, you could win $1 million! Earlier this month, Governor Cooper announced the $4 Million Summer Cash drawing – an opportunity for four vaccinated adults to win $1 million each. All North Carolina residents who have been vaccinated with at least one dose are automatically eligible. Those vaccinated on or after June 10 will be entered twice for each drawing. The drawing begins today, with winners announced biweekly on Wednesdays through August 4. Get vaccinated early to increase your chances of winning.

Next step: Complete the vaccine survey
Complete the vaccine survey at your earliest convenience, and no later than August 2. If you have been vaccinated, please have your vaccine details readily available prior to accessing the survey. Regardless of your current vaccination status, your participation in this process is critical to evaluating our preparedness for a fully operational campus this fall. If you have trouble accessing the vaccine survey, copy and paste the following URL into your browser: https://wakeforest.qualtrics.com/jfe/form/SV_2otFrmdwhtwynFI

Thank you for joining colleagues across campus in providing this essential information and for promoting a safer and fully operational campus environment this fall. If you have questions or feedback, please contact Human Resources by using the HR Feedback Form or emailing askHR@wfu.edu.

Sincerely,
Wake Forest University Human Resources
