The following message was sent to faculty and staff who need to upload their vaccination documentation directly into Workday. 

Dear Colleague,

Thank you for providing your vaccination status to the university. A fully vaccinated campus is the best path back to full operations and a reconnection with students and colleagues.

It is now time to provide your proof of vaccination documentation to the university. To complete this process view this COVID-19 vaccination documentation tutorial and access the task in your Workday inbox.

Important: Have a digital copy of your vaccination documentation ready for upload before beginning this process. The easiest vaccine documentation to provide is either a photo or a scan of the front of your vaccine card that includes your name, the vaccine name, and the dates of each dose. The Faculty and Staff Clinic is available if you have lost your vaccination card and need help accessing your North Carolina COVID-19 vaccination record; please call (743) 223-4217 for assistance. If you were vaccinated outside of North Carolina and have lost your vaccine card, you will need to contact the state health department in which you were vaccinated directly for your record. It’s important to note that if you upload multiple documents, previous files will be replaced and only the last uploaded document will be available. If your vaccination documentation is in multiple files, please combine them into one PDF before uploading.
