Healthy Outcomes Program Activities

Wake Forest Reynolda Campus medical plan participants may complete the activities below to earn credits as part of the Healthy Outcomes program.  If you earn at least 120 credits by March 31, 2015, you may receive a well-being incentiveView > Healthy Outcomes Presentation


Health Assessment
(Required, once per year)

The online Health Assessment helps gauge your current health status, so you can track your progress.  It takes approximately 15 minutes to complete and helps you:

  • Identify health concerns
  • Discover relevant programs
  • Find recommendations for stress relief, fitness, better nutrition, and more

Healthy Living Programs
(20 credits each, maximum of two)

The six-week online Healthy Living Programs cover a variety of topics, including:

  • Smoking cessation
  • Stress relief
  • Nutrition
  • Fitness
  • Conditions such as diabetes and heart disease

Throughout the programs, you receive weekly to-do lists, e-mail reminders, tips, and other valuable information on the topic you choose. Read more »


Healthy Living Conversations
 (10 credits each, maximum of three)

The interactive Healthy Living Conversations cover a variety of health topics, such as:

  • Healthy eating
  • Back pain
  • Fitness
  • Sleeping well

Your responses tailor each conversation to your needs.  Each conversation takes approximately 10 – 15 minutes to complete. Read more »


Monthly Seminars
(five credits each, maximum of nine)

Fun and engaging online seminars help you learn more about important health topics, such as nutrition, fitness, stress relief, and more.  The seminars last approximately 10 – 15 minutes.


Preventive Exams
(50 credits each, maximum of one)

You may earn credits for being current on the following preventive care screenings/exams (if applicable):

  • Breast cancer screening (women ages 40 – 69): Every 12 – 24 months
  • Cervical cancer screening (women ages 21 – 64): Every 36 months
  • Colorectal cancer screening (adults ages 50 – 75): Fecal occult blood test every year, sigmoidoscopy every five years, or colonoscopy every 10 years
  • Physical exam (adults ages 19 – 39): Every 36 months; (adults 40 or older): Every 12 months
