Annual Salary Administration Form

All employment or position changes, with a July 1 effective date, must be completed via the Salary Administration Form. If paperwork for employment or position changes had previously been submitted to Human Resources, before the Annual Salary Administration process, this form will not need to be completed.

Changes include:

  1. Additional compensation such as a performance bonus or contractual obligation
  2. Budget code or funding source change from one department to another
  3. Change in budgeted work hours
  4. Change in exemption status (monthly to biweekly paid or vice versa)
  5. Promotions or Transfers
  6. Start, change, or end of Administrative, Chair, Directorship, or Professorship Stipends
  7. Title Changes
  8. Termination, exit, or retirement of faculty or staff member effective June 30, 2016
